00:25 treatments that you can start doing today, at home,

00:48 My name is Nick Evanson and you are watching the
00:50 Ultimate Prescription and we are also here with Dr. James
00:53 Marcum and we work together at Heartwise Ministries.
00:56 We are non-profit that has the goal of bringing people
00:59 closer to Christ through following His plan
01:02 for their life. Right! You know, if you look at Christ's
01:04 model, especially in Luke for instance.
01:07 He met their needs, he tried to meet their physical needs,
01:11 He healed them, He loved them, He didn't judge them,
01:15 and once that physical need was met, he introduced them to a
01:18 relationship with the Heavenly Father and he brought them
01:22 into that growing grace. Um, and that is what we try to do
01:26 at Heartwise, and then we try to meet someone's physical need
01:29 and teach them how to move forward one step at a time.
01:32 Not only towards health and healing,
01:33 but also in their relationship with God, we encourage that.
01:37 encourage them to come to worship.
01:39 Encourage them to understand that God is there for them,
01:42 and wants to be their ultimate physician.
01:44 Also on earth you know, we are probably our own best
01:48 physicians and let them know the place for modern medicine.
01:52 which is to treat acute problems and chronic problems
01:56 which is to try to get back to the way our body was created
01:59 in the beginning. I know we have been talking a lot about
02:02 pain and we are going to try to sum up and answer some
02:05 questions we haven't answered, but where do you think we should
02:08 start today? Well, I am going to go through
02:10 and kind of quiz you on the different types of pain
02:12 people might be experiencing at home and you can kind of
02:15 tell us what type of physician they should go see. Yeah,
02:17 You tell me the type of pain and I will the type of specialist
02:21 that you should see. I'll also mention the most
02:24 dangerous thing about that type of pain in that part
02:27 of the body. So we are going to start off
02:28 with pain in the mouth, oral pain.
02:30 Ok, oral pain. Two sets of doctors would be very helpful
02:34 for that type of pain. One is a dentist because they are in
02:37 the mouth all the time, and the other one that gets
02:39 called in all the time is an E.N.T., Ears, Nose, and Throat.
02:43 They are specialists that deal with the ear, nose, and throat
02:46 and that typically has oral pain. Um, there is not a lot
02:51 of oral pain that I see kill people right away, so that's
02:54 good. Right! You know, they don't have to run to the
02:56 Emergency Room all the time right away, so those are the
02:59 two types of doctors for that type of pain.
03:01 Alright! How about if someone is experiencing abdominal pain.
03:04 Abdominal pain could be a medical emergency,
03:08 especially if we rupture our big tube, if we are bleeding
03:12 inside. So if the pain is rapid and won't go away
03:15 associated with a fever, the belly is tight, nausea,
03:19 lots of diarrhea, you probably need to go to the Emergency
03:22 Room right now. ok. The specialists that deal
03:24 with that pain is usually Gastroenterologists,
03:28 GI specialist. They work with that pain frequently
03:32 um, if a blood vessel is involved they might do that.
03:34 General Surgeon's also work with that type of pain
03:37 quite a bit in the abdomen. But you want to make sure
03:40 that it is not heart pain either because sometimes
03:43 heart pain can simulate belly pain in some instances.
03:46 Alright, what about pain of the sex organs or down in the
03:50 private parts. What kind of doctor should you see?
03:52 Well, genital pain is either a Urologist or a Gynecologist.
03:58 Those are the doctor's that really specialize in that
04:00 type of pain. Now a good provider or good
04:04 family physician, a good internist, a good nurse
04:07 practioner, all of them might be able to help 90% of that type
04:10 of pain. But I am just talking for the 10% that might need
04:14 a specialist. Right! What about the nervous system.
04:17 There can be a lot of complex issues with the nervous system,
04:20 who do they see? Well, the Neurologist.
04:24 Neurologists are very good at doing that. They are experts
04:29 at evaluating the neurological system when your regular doctor
04:34 needs a little extra help. Like I said, a lot of these
04:37 conditions most doctors can handle pretty competently,
04:41 and you only need a specialist maybe 10 or 20% of the time.
04:45 What about joints and muscle aches?
04:48 Orthopedic doctors are very good to go to.
04:51 Sometimes good Physical Therapists and good
04:54 Chiropractors are other alternatives depending on the
04:57 nature of the joint pain and what type of professionals
05:00 you have in your area.
05:01 What about if someone is having chronic headaches and
05:04 that type of thing, who should they see?
05:05 That's another one, Neurologist is usually the one
05:07 to go for a chronic headache or a good Internal Medicine
05:10 doctor because an Internal Medicine doctor sort of looks
05:12 at everything. Those are some specialists that might evaluate
05:15 that type of pain.
05:16 Ok. The last one here I have on my list is going to be pretty
05:19 easy for you, someone having chest pains, who do they go see?
05:22 I think they need to see a "Cardiologist" if you are having
05:25 chest pain. It is different if someone hits you in the chest
05:29 and you know what is causing it, that's different but
05:31 for pain that is undiagnosed, you don't know what it is
05:35 a "Cardiologist" and any pain that is above the waist
05:39 that is unexplained, that is ongoing, I think you need
05:42 evaluation right now, it can't wait because it could be a
05:45 heart attack and then any pain that happens when you are
05:49 walking and doing something on exertion, that's when the heart
05:51 works, that might be very also suggestive of heart.
05:55 The reason I bring this up is because the heart pain
05:57 is probably the most common cause of acute pain
06:01 that's out there. It is the most common cause of death
06:04 in the United States, it's the most common cause of
06:07 lots of problems, so I am very sensitive to heart pain.
06:10 Yep, yep! Well on today's program

06:30 I want to talk for just a second why pain is so important.
06:34 Often times it is thought of as a really negative thing
06:38 and it is, certainly if your spirits are chronically.
06:41 but pain is kind of a gift that God gave our bodies
06:46 to notify us that something is wrong.
06:48 So why is it so important? That is so important,
06:51 Because how would you know that something was wrong
06:53 if you didn't have a signaling system.
06:55 Right! You would never know. It's like a true North,
06:58 You know if you are doing well you are not having pain.
07:00 Well things might be pretty good because God made us
07:05 to have pain when things aren't going well,
07:07 so that's why it's so important. Also if your having pain,
07:11 pain is a form of stress on the body.
07:13 It's great if you need to warn yourself to get help right away,
07:17 that's what it's for, but pain long term, turns on stressors
07:22 on the body which can damage it further, so chronic pain
07:25 is not good, it makes the adrenalin go up,
07:28 you don't think as good, your cognition,
07:30 the way you figure things out aren't as good,
07:33 people in chronic pain, they make mistakes easier at work,
07:38 they can't figure out simple things sometimes,
07:40 so chronic pain really affects not only the part that's
07:43 having the pain, but indirectly it affects your entire body.
07:47 Chronic pain even affects the way you worship,
07:50 people in chronic pain can't worship as well as people
07:53 that don't have chronic pain because of the brain
07:55 and how it is reacting and all the stress chemistry
07:58 that's turned on. God understands this and
08:01 He works with us, but that is some of the two biggest
08:04 reasons pain is important is that it is an alarm system
08:07 that something is not right, and two a longstanding pain
08:11 is dangerous to the body.
08:12 That's why there are some instances if you can't figure
08:14 out what's causing the pain, the pain needs to be treated
08:17 because the damage from the pain is probably more risky
08:20 to you than the treatment for the pain.
08:22 Alright. That makes a lot of sense to me. Yeah!
08:25 Good. Let's talk about what to do when someone's having pain.
08:29 We just talked about the lists, the big lists, the different
08:31 types of pain and who people should see, but I know
08:34 a lot of folks a tendency to just try to tough it out
08:36 and I try and do the same thing, why is that a bad idea?
08:39 Because it could kill you. Ok. You could die if your
08:42 judgment is wrong.
08:44 I've seen people through my life that have chest pain
08:47 they think it's nothing serious, they stay at home,
08:49 they have a heart attack, an arrhythmia and it's over with.
08:52 Half the time, the first time we know that someone's having
08:55 a heart attack is when... I mean we know that they have
08:58 cardiovascular disease is when they have a heart attack.
09:01 It's a disease, and it causes sudden death.
09:05 Let's say they can't breathe, you are all of a sudden
09:09 real short of breath. That could be a "pulmonary embolism".
09:11 Let's say you "pass out", that could be a sign that
09:15 the blood vessel or there is something wrong with your brain.
09:17 Now those aren't pains, but those are dramatic things
09:20 that happen. The severe headache, the worse headache
09:23 in your life, if you can't explain it, that can be a
09:25 broken blood vessel which which could be life threatening.
09:28 all of a sudden your belly starts hurting and it gets
09:31 bigger and you get nauseated. Well that could be a serious
09:35 sign that there is something wrong with your belly.
09:37 So pain in those areas could be a sign that something is
09:41 threatening your life, so unless you know it is not
09:44 life threatening, you need to get to someone to see if it
09:47 is life threatening and give you some advice,
09:49 and help you go from there. So pain happens for a reason.
09:53 Now earlier in the series, we talked about bones and you
09:58 mentioned that bone health, or bone pain specifically,
10:02 can be very, very... one of the worse pains that someone
10:05 could experience. Why is that?
10:06 Well bones have lots of nerves in them, you know, there's
10:09 lots of things going on in the bone and when a bone is broken,
10:12 the nerves, blood supply, that's altered as well.
10:15 Have you ever broken a bone? Not yet! Ok!
10:18 Have you ever chipped a tooth? No, I never actually chipped
10:22 a tooth? You never chipped a tooth. Man, you're not helping
10:24 me out here. Well you know if you chip a tooth or break
10:27 a bone, the air hits the tooth and you know it gives you
10:30 that chilling. The nerve is exposed right away,
10:32 and it makes this terrible pain,
10:34 and some of the worse pain there is is when a malignancy has
10:38 metastasized to the bone and cancer gets in the bones,
10:42 that's some of the worse and severe pain there is.
10:45 Trauma, like a break in the bone, that is severe pain.
10:49 When a point tenders and you touch it here, Ohhh!
10:52 It hurts, well, that's because there is such extreme pain
10:55 and the nerves are so sensitive to bone pains.
10:57 Why those nerves seem to be extra sensitive
11:00 well bones are so important, they are structured
11:03 the backbone of us...You know a little piece of skin that's
11:06 not that important, but if your bone breaks, that's a
11:08 big deal, you know your whole system, that's the foundation,
11:12 that's the boards of your body. Right! So that's a big deal
11:15 with that. Infection in the bones, that seems to hurt
11:18 a whole lot, so those are some of the causes of bone pain
11:22 and bone pain really hurts a lot and we can sort of sort
11:25 through this with modern medicine,
11:26 what's causing that pain.
11:28 Sure. This next question is one I can relate to,
11:31 I've experienced this, and that's a question about
11:34 ligaments and tendons. I've had issues with both personally
11:38 but what are these parts and what are they important,
11:41 and why do they cause pain?
11:43 A lot of people don't know... We talked about the bones
11:46 okay, the bone is sort of the skeleton in the body,
11:49 we look at those with x-rays. Now a ligament, okay, a ligament
11:56 is the tissue that connects the bones to the joint.
12:00 The ligaments, bones to joints okay, and what the tendons
12:05 connect muscles to bones, so I would think of ligaments
12:09 and tendons as things that connect things together.
12:11 Bones to joints, muscles to bones, things of that nature,
12:16 their connectors. If you have had... You know every question
12:21 I have asked you, you said no. You didn't hurt your teeth,
12:25 and you didn't break a bone. Did you ever play with
12:27 Lincoln Logs? Oh sure! And Tinker Toys? Yeah, Um hum.
12:31 Good. Okay. You know how you have those Tinker Toys you have
12:33 these little things that connect things together. Yeah! Yeah!
12:36 Sort of like ligaments and muscles. Well whenever these
12:40 things get lax, the bone is affected because the bone
12:44 changes position and you could have inflammation when all this
12:47 happens. The bone because of repetitive movement,
12:50 you know remember all these move the body and design us
12:53 to even do things. If they are not in the right positions
12:56 guess what else is not in the right position?
12:58 Mr. Nerves too. The nerves too. The nerve goes hey,
13:02 this ligament and this tendon are laxed, they are inflamed
13:05 the bone is not happy, I am not happy either.
13:08 So I am going to tell Mr. Brain that I am having pain and
13:11 something's wrong. Right!
13:13 So those are some things that all sort of involve
13:15 and it is so critical because that's what keeps us moving
13:18 and doing things. That sort of like the backbone of our bodies
13:21 so that's what ligaments and tendons that connect bones
13:24 and muscles to different things.
